Please be aware that our clinic operates on a self-pay basis and does not accept insurance for most services. Currently, we accept PPO insurance only for lab bloodwork. Patients who wish to utilize their PPO insurance for lab work must present their insurance card in person. We will make a copy of the card for the lab’s use.
Since we do not have a billing department, you are responsible for managing any costs not covered by your insurance. We do not handle insurance billing or reimbursement directly. Any financial responsibility for services not covered by your insurance is solely yours.
Our services are provided "as is" and "as available," without warranties of any kind. We do not guarantee the accuracy or reliability of the results obtained from the use of our services.
9. Limitation of Liability
